Hiperparatiroidizm Nedeni İle Ameliyat Edilen Hastalarımızın Sonuçları

Öz


Özet Hiperparatiroidizm nedeni ile 1998–2006 aras›nda Trakya Üniversitesi T›p Fakültesi Genel Cerrahi Anabilim Dal›nda ameliyat edilen 36 hastan›n sonuçlar› retrospektif olarak de¤erlendirildi. Hastalar›n 26’s› (% 72) kad›n 10’u (% 28) erkekti. Hastalar›n yafl ortalamas› 46.6 (17–75) idi. Hastalar›n baflvuru flikayetleri, kemik ve eklem a¤r›lar›, osteoporoz, dizüri, dispepsi ve hipertansiyon idi. Preoperatif tan› amaçl› tüm hastalara boyun ultrasonografisi ve 99mTc-M‹B‹ ile paratiroit sintigrafisi, 7 (% 19) hastaya manyetik rezonans görüntülemesi, 4 (% 11) hastaya da boyun bilgisayarl› tomografisi yap›ld›. Tüm hastalar›n alkalen fosfataz, fosfor, kalsiyum, parathormon de¤erlerine bak›ld›. Bilateral boyun eksplorasyonu yap›lan hastalar›m›zdan biri, nüks hiperparatiroidi nedeniyle reopere edildi. Hiperparatiroidi tahmin edilenden çok daha s›k rastlan›lan ve kolay tan› konabilen bir hastal›k olmas›na ra¤men klinisyenler taraf›ndan ayn› kolayl›kla gözden kaçabilmektedir. Hiperparatiroidi hastalar› çok çeflitli flekillerde belirti vermeleri nedeniyle as›l tedavi edilecekleri klinikler d›fl›nda gereksiz olarak tetkik ve tedavi edilmeye çal›fl›l›rlar. Hiperparatiroidi hastalar›n›n genel cerrahi, endokrinoloji, nükleer t›p ve radyoloji uzmanlar›n›n oluflturdu¤u bir ekip taraf›ndan multidisipliner anlay›flla de¤erlendirilmeleri ve tedavi edilmeleri ideal yaklafl›md›r. . The outcomes of our patients operated for hyperparathyroidism

Abstract In this study, 36 patients who underwent parathyroidectomy operation due to hyperparathyroidism between 1998- 2006 in Trakya University Medical Faculty Department of General Surgery were retrospectively evaluated. There were 26 (72 %) female and 10 (28 %) male patients. The mean age was 46.6 (17-75 years). The complaints of the patients were bone and/or joint pains, osteoporosis, dysuria, dyspepsia, and hypertension. Preoperative diagnosis was made in all of the patients thorough the neck ultrasonographic evaluation and 99mTc-M‹B‹ parathyroid scintigraphy. Another 7 (19 %) patients had an additional magnetic resonance imaging scan, and 4 (11 % 9) had an additional computed tomography scan of the neck. The levels of se- rum calcium, phosphorus, parathormone, alkaline phosphatase were investigated in all of the patients. All of the patients under- went bilateral neck exploration and one patient was reoperated due to recurrent disease. Although hyperparathyroidism is such an easy diagnosed and frequently seen illness, many clinicians dismiss this pathology. Patients with hyperparathyroidism tried to be investigated and treated in different clinics due to the large variety of symptoms. However patients with hyperparathyroidism should be treated by a multidisciplinary team consisting of general surgeon, endocrinologist, nuclear medicine specialist, and radiologist.
